Your part of the form is easy and brief, less than a page. Block A is “Vehicle Description and Applicant Information,” Block D is “Optional Personalization Request,” and Block E is your signature and date. Mail the completed form to us with the fee, and we’ll do the rest.

Request the application from PSNA, complete it, and mail it back to us with the required fee. We will approve, process, and send the application to PennDOT. Special organization plates are available for passenger cars, motor homes, and trucks with a registered gross weight of 9,000 pounds or less. Plates are issued in numerical sequence or, for an additional fee, can be personalized with up to five letters or numbers, subject to availability and approval. Plan on 6 weeks for the state to get you the plate. PennDOT’s been great, so it might even be 4 or 5 weeks. Whatever the time, it’s worth the wait!
Choose from two prices payable to PSNA at the time of application: $100 for a non-personalized plate or $236 for a personalized plate. Of those amounts, PennDOT receives a special organization issuance fee of either $34 (non-personalized plate) or $170 (personalized plate); in addition, Pennsylvania Correctional Industries receives $6 for each plate. PSNA and its supporting foundation use the remaining $60 to fund nursing education scholarships and to cover administrative costs.
The Good Samaritan Act protects healthcare workers if they stop at the scene of an accident. However, it does not demand that they stop.
